Fort Hunter sits about five miles west of Amsterdam, just off Route 5S, where Schoharie Creek runs into the Mohawk River. When someone here starts looking for a local rehab center, it is usually for a spouse, a parent, or themselves. Most of the time, something specific happened that made waiting feel impossible, and what they want now is a clear next step.
Calls from Fort Hunter tend to follow a relapse, a hospital discharge, or the day someone finally says yes to help. Conifer Park has no location in the hamlet, so most families work with our Schenectady outpatient clinic or our inpatient campus in Glenville, both reached by heading east on Route 5S.
You do not need a referral or a diagnosis to call. Many of the people who reach us are family members trying to work out what to say to someone who has not agreed to treatment yet.
We have treated substance use disorder in New York since 1983, and we are licensed by the New York State Office of Addiction Services and Supports (OASAS). Substance use disorder is a medical condition where a person cannot control their use of alcohol or drugs despite the harm it causes. For many Fort Hunter families, outpatient treatment is the starting point. That means regular sessions while living at home, with counseling such as relapse prevention, anger management, and family therapy.
Some people need a higher level of care before outpatient makes sense. Our inpatient rehab program at the Glenville campus is residential care with structured therapy every day. It often follows medically supervised detox, where the body clears a substance under medical monitoring. Detox can include medication-assisted treatment (MAT), which pairs approved medications such as Vivitrol, methadone, or buprenorphine with counseling.
With no walk-in option nearby, almost everything starts with a phone call. Admissions staff ask a few questions, go over which insurance is accepted, and set up an assessment, so nobody has to drive anywhere before they know what comes next.
For inpatient and detox admissions, we provide free transportation from anywhere in New York State, arranged after intake, with room for two suitcases per patient. The admissions line is answered 24 hours a day whenever you are ready to talk it through.
Each person is assigned one case manager who stays with them for the length of the program. Aftercare planning, the plan for ongoing support once a program ends, starts before discharge rather than after it.
We accept Medicaid, Healthfirst, Excellus Blue Cross Blue Shield, Fidelis Care, MetroPlusHealth, MVP Healthcare, and most major commercial plans, and self pay agreements are available based on need. Privacy matters in a hamlet where neighbors know each other. Conifer Park operates fully confidential addiction treatment programs under federal 42 CFR Part 2 regulations. This means the facility cannot confirm or deny any person's participation in its programs without that person's written consent.
Fort Hunter is a Montgomery County hamlet at the meeting point of Schoharie Creek and the Mohawk River. It is home to the Schoharie Crossing State Historic Site, where remains of the original Erie Canal still stand. Route 5S runs along the south side of the river and links the hamlet east to Amsterdam and on toward Schenectady, which is the most direct way to our Schenectady clinic and the Glenville campus.
