Benefits for Full Time Employees

Group Health Insurance – Blue Shield of North Eastern New York. Prescription coverage and limited vision coverage included (see separate rate sheet).

Dental Insurance – Preventive, basic, and major restorative and orthodontia services. In network and out-of-network services (see separate rate sheet).

Vision Service Plan –  Exam lenses/contacts covered in full every twelve (12) months; frames every twenty-four (24) months. Discounts and savings on laser vision correction, scratch resistant, anti-reflective coatings and progressives. Co-pays apply. Greater savings when going to a participating doctor. Some exclusions apply (see separate rate sheet).

Life Insurance – Coverage equal to one times annual salary rounded to nearest thousand. Company paid. Portable upon termination. Rate based upon age and annual salary.

Voluntary Term Life Insurance – May purchase insurance equal to 1X, 2X, or 3X annual salary for self. Ability to purchase for family members not greater than 50% of amount purchased for self. Maximum $300,000. Non-medical limit for spouse $35,000. If election above $35,000, evidence of insurance required. Unmarried children from 14 days to 19 years old $10,000 maximum (see booklet exceptions). Portable upon termination. Rate based upon age and salary.

Accidental Death and Dismemberment – Coverage equal to 1X annual salary. Cost based on annual rate of pay. Coverage reduces as you age. (See separate sheet)

AFLAC Accident-Only Insurance – Payable for a covered person’s death, dismemberment or injury caused by a covered accident. Available through age 64 and may also be purchased for dependents. Portable upon termination. (See an Aflac representative to learn more or elect.)

AFLAC Personal Cancer Indemnity – Pays benefits directly to you, unless otherwise directed. You use the cash however you decide. There is a 30-day waiting period and may also be purchased for dependents. (See an Aflac representative to learn more or elect.)

AFLAC Short-Term Disability – Pays benefits regardless of any other insurance & provides a source of income while recuperating from a covered injury/illness. Must work 30 hours or more weekly and annually make a minimum of $17,000. (See an Aflac representative to learn more or elect.)

AFLAC Lump Sum Critical Illness – Options are 10,20 and 30k. Cash goes directly to policyholder and is guaranteed renewable for your lifetime. Applies when policy holder has heart attack, stroke, end stage renal disease, or major organ transplant (see an AFLAC representative to learn more or elect).

Long Term Disability – Benefit paid when disability is caused by a disease qr accidental bodily injury occurs. Benefit starts after six (6) months of disability. Payment of 60% of gross wages to a maximum of $10,000. Coverage reduces as you age. (Sec separate sheet).

Disability Insurance – After four (4) weeks of employment. Lost wage coverage for non-job related lost time injury or illness. Maximum $170.00 weekly.

Workers Compensation – Upon employment. Medical and lost wage coverage for any job related injury or illness.

* *401(K) Plan – Eligible to participate the first of the month after ninety (90) days of employment. Permitted to contribute up to 60% annually on a pre-tax basis. Maximum annual contribution is $17,000. Non­ union employees, see an HR representative.

Paid Time Off – See separate sheet.

Holidays – There are nine (9) recognized holidays: New Year’s Day; Martin Luther King Day, President’s Day, Memorial Day, Independence Day, Labor Day, Columbus Day, Thanksgiving Day and Christmas. (See Article 16 of Union Contract for additional info)

Flexible Spending / Dependent Care Account – Allows you to use pre-tax dollars to pay for certain expenses/dependent care. FSA annual maximum $2,500. DCA annual maximum $5,000. Use it or lose it provision applies.

Tuition Assistance – Eligible after one (1) year of employment. Maximum reimbursement of $1,200.

Pension Plan (Union Employees) – FT and benefit eligible PT union employees upon completion of one (1) full year of employment but less than five (5) years will have a contribution of a minimum of $.50 per hour or 3.25% of the employees hourly wages, whichever is higher for time actually paid, per employee up to 40 hours per week, with a limit of 2080 hours per year.

Upon completion of five (5) years, a contribution of $.85 per hour or 6% (in addition to the above stipulations).

Direct Deposit – Full or partial paycheck deposits to local banks and credit unions upon employment. Should you separate employment, your last check is not direct deposited.

Meals – Lunch is served daily at a cost of $2.50. Purchase 5 meal tickets and pay only $10.00. While working, beverages are free. Snack day is provided on Wednesday at no cost.

Parking – Upon employment. Free parking.
