Conifer Park, Inc. offers a very generous paid-time-off package. Time off is pro-rated based upon date of hire. Union employees employed as of the first of the year are eligible for 9 holidays, 2 personal days, a total of 10 sick days, 7 floating holidays and 10 days of vacation. Based upon years of service, the maximum amount of vacation time off is 25 days. Part-time employees (20 hours to 31 hours), are also eligible for many benefits on a pro-rated basis.

PHYSICIAN ASSISTANT or NURSE PRACTITIONER
Location: Glenville, NY
FT/40 hours 

Develops and implements patient management plans, medication and therapy recommendations, records progress notes, and assists planning of continuity of care. Performs history and physical examinations on patients. Evaluates and manages detox protocols on the Detox Unit. Pre-Admission clearance for patients with medical symptoms and Pre-Admission Interviews when indicated.

Requirements:  Must have current NYS License and Physician Assistant certificate from National Certification Commission /current NYS NP license. 1 – 3 years experience as a physician assistant/nurse practitioner; Experience in detoxification / addiction medicine exp a plus.

 

EVENING NURSE SUPERVISOR
Location: Glenville, NY
Full time 40hrs Monday-Friday 

Under the direction of the Director of Nursing:

  • Provides Nursing expertise and education to all Nursing staff, including orientation of new hires and includes supervisory responsibilities
  • Identifies staff instructional needs and provide teaching.
  • Provides patient education classes for smoking cessation.
  • Assists staffing coordinator with staffing and scheduling.
  • Manages material resources in a cost effective manner
  • Ensures adequate medical supplies, narcotics and over the counter medications.

Requirements: AAS required, 4 year/ Bachelor’s Degree preferred. A valid NYS RN License. 1-3 years supervisory experience and CPR Certification.

 

REGISTERED NURSE
Location: Glenville, NY
Part time all shifts and Per Diem all shifts (as needed)

Performs assessments focusing on physiologic and cognitive status. Formulates a goal-directed plan of care. Evaluates effectiveness of care and patient progress toward outcomes.

Requirements: Associate Degree, current NYS RN license w/1-3 yrs exp and CPR certification required.

 

ADMISSION RN
Location: Glenville, NY                                                                                                                                            FT/40   Monday-Friday   3:00p-11:30p                                                                                                       

Assess candidates and facilitates admission into various treatment programs.  As required, communicates with insurance companies/managed care to pre-certify patients for admission. Arranges private payment plans with family and/or referent.  Works with team to field all inquiries, informational calls and admissions.  Completes nursing assessments, obtains orders from a provider and completes OASAS admission forms as needed.

Requirements: Must be a RN in NYS and have 1-3 years experience in Chemical Dependency, managed care or case management.

 

LICENSED PRACTICAL NURSE
Location: Glenville, NY
Part time days & evenings and Per Diem all shifts (as needed)

Collaborates with the RN in the implementation of a total health care regime including participation w/ patient and family teaching including discharge planning. Provides data to assist in the evaluation of patients’ responses to plan of care. Med pass.

Requirements: Vocational/Technical School w/ 1-3 yrs exp. Current NYS LPN license and CPR certification required. knowledge/experience.  Individual and Group counseling as well as didactic education skills required.

 

ADDICTION SPECIALIST
Location: Glenville, NY
Full Time 40hours

Assumes full charge of patient case management, delivering individual and group mental health and chemical dependency services including assessment, discharge planning, managed care, and relevant communication, formulation and implementation of treatment plans, and performing specified duties.

Requirements:  Bachelor non-licensable degree and CASAC OR Master’s licensable degree w/1 yr relevant experience OR Master’s licensed degree w/QHP. 1-3 years experience in chemical dependency with mental health knowledge/experience.  Individual and Group counseling as well as didactic education skills required.

 

ADDICTION TECHNICIAN
Location: Glenville, NY
Part time and Per Diem all shifts (as needed)

Monitors, oversees, and escorts patients. Review notes and appropriate documentation on patients on a daily basis helping to ensure continuity and compliance with treatment. Provide psycho-education services. Monitors and enforces the facility policy as it relates to patient behavioral management issues.

Requirements: HSD or GED. 1-3 yrs experience in the Human Services or Health Care field, including positions requiring patient contact and general use of appropriate interpersonal skills including patient to staff boundaries and management of patient/client groups. Associates/Bachelor’s Degree may be substituted for the experience requirement. NOTE: some positions require CPR.

 

ADMISSIONS SPECIALIST
Location: Glenville, NY
Full time 40 hrs 

To coordinate and perform all tasks required to prepare a patient for an admission.

Requirements: 2 year/Associate’s Degree and 1 – 3 years in Chemical Dependency or in an office environment.

 

RECEPTIONIST
Location: Glenville, NY
FT/40 hrs 12:00pm-8:30pm Monday-Friday

Answers and screens calls. Greets all visitors to the facility, follows confidentiality procedure and high standard of customer service.

Requirements: HSD/GED PLUS 1-3 months experience reception and clerical experience. Good communication skills

 

INSUARANCE/PRECERT TECH
Location: Glenville, NY
FT 40hrs Monday – Friday 8:00a-4:30

Input admissions, transfers and discharges in the computer system. Input all ancillary charges, post payments from patients and insurance companies to their respective accounts. Calculate daily census. Run daily reports.

Requirements:  HSD or GED required. 2 yr/Associate degree preferred 1 – 3 years experience in Insurance and/or billing experience required.

 

HOUSEKEEPER
Location: Glenville, NY
FT/40hrs 7:00-3:30p (2 positions available)

Performs the day to day housekeeping and laundry functions necessary to provide the goals of a clean healthy environment in accordance with the department of Health, Safety, and Departmental regulations guidelines.

Requirements: HSD or GED preferred. 1 – 3 years experience preferred.

 

DIETARY ASSISTANT
Location: Glenville, NY
Part Time 20 hours with benefits

Assist in preparation and service of all aspects of the Dietary Department. Cleans, stocks and organizes service line, coolers and storage areas.  Takes inventory and assembles supplies for all unit kitchens.  Follows all mandates concerning food preparation, services, storage and sanitation as set forth by the New York State Department of Health

Requirements: Less than HSD; HSD or GED preferred. Vocational/Technical in Food services a plus. No experience required, experience in food service and/or food preparation a plus. Must be able to follow Department of Health regulations and safety guidelines.

 

OUTPATIENT CLINICS:

REGISTERD NURSE or LPN- PLATTSBURGH
PT/20hrs with Benefits

Provide comprehensive psychosocial assessment and diagnostic services to patient in an outpatient setting. Provide comprehensive case management services and coordinate services with collateral service providers. Provide comprehensive and individualized discharge planning and referral services.

Requirements: NY State RN License. 1 – 3 years Case Management experience in Chemical Dependency. CPR Certification

 

OUTPATIENT COUNSELOR- LIVERPOOL
Full Time 40 hours

Provide comprehensive psychosocial assessment and diagnostic services to patients in an outpatient setting. Provide comprehensive case management services and coordinate services with collateral service providers. Provide individual, family or crisis counseling to a caseload of up to 35 clients.

Requirements:  HSD or GED required. Must have a CASAC-T or a provisional MHC. 2yr Degree in Chemical Dependency or 4yr BA/BS in related filed preferred. 1 – 3 years experience in counseling preferred. Commitment to obtain QHP within 5 years of hire. CPR required within 6 months of hire.

 

OUTPATIENT PRECERT/EVAL RN- LIVERPOOL
Full Time 40 hours

Triages for appropriate placement/admission and makes appropriate recommendations of patients. Meet with patient to collect data in preparation for Insurance pre-certification call. Communicates with insurance companies/managed, professional review organizations, union to pre-certify patients for admission.

Requirements:  Bachelor’s Degree Preferred.  Appropriately licensed RN with 1 year experience in Managed Care.

 

SECRETARY
Full Time 40 hours

Provide administrative support to the outpatient staff. Answer telephones, screens calls, and responds to requests from patients and staff. Makes and confirms appointments, schedules and coordinates special meetings.

Requirements: HSD or GED. 1-3 years clerical experience. Must be proficient in typing and have excellent communication and computer skills required.  CPR is required within 6 months of hire.
