Conifer Park, Inc. offers a very generous paid-time-off package. Time off is pro-rated based upon date of hire. Union employees employed as of the first of the year are eligible for 9 holidays, 2 personal days, a total of 10 sick days, 7 floating holidays and 10 days of vacation. Based upon years of service, the maximum amount of vacation time off is 25 days. Part-time employees (20 hours to 31 hours), are also eligible for many benefits on a pro-rated basis.

TALENT ACQUISITION SPECIALIST
Location: Glenville or Carmel, NY
Full Time 40 hours

Our Talent Acquisition Specialist will be responsible for all recruitment functions for both facilities in a unionized environment.  Duties will include designing job postings, screening resumes, interviewing candidates, developing relationships with area colleges and extending employment offers.   

Requirements:  The qualified applicant will have an Associate’s Degree, Bachelor Degree preferred, with 3-5 years HR experience.  Strong recruiting background required plus the ability to multi-task, exercise good judgement and discretion, and maintain a high degree of confidentiality.  Good interpersonal skills and ability to work with all levels of management and employees also required. Strong Microsoft office skills required; on-line applicant tracking systems experience is a plus.

 

PHYSICIAN ASSISTANT or NURSE PRACTITIONER
Location: Glenville, NY
Full Time 40 hours, Part time and per diem (as needed)

Develops and implements patient management plans, medication and therapy recommendations, records progress notes, and assists planning of continuity of care. Performs history and physical examinations on patients. Evaluates and manages detox protocols on the Detox Unit. Pre-Admission clearance for patients with medical symptoms and Pre-Admission Interviews when indicated.

Requirements:  Must have current NYS License and Physician Assistant certificate from National Certification Commission /current NYS NP license with a minimum 1 year of experience as a physician assistant/nurse practitioner; Experience in detoxification / addiction medicine exp a plus. 

 


REGISTERED NURSE

Location: Glenville, NY
Part time day / evening and Per Diem all shifts (as needed)

Develops an individualized plan of care based on data obtained from all available sources. Implements nursing and medical orders in accordance with established standards, policies and procedures. Evaluates and reviews effectiveness of interventions and implementation of nursing care. Assumes charge duties of nursing unit under the direction of the Nurse Manager. Works closely with Medical and Clinical Team.     

Requirements: Associate Degree, current NYS RN license with a minimum 1 year of experience. CPR certification required.


ADMISSION RN
Location: Glenville, NY
Full time 40 hours Tuesday-Saturday 9:30am-6:00pm                                                                                                                                      

Assess candidates and facilitates admission into various treatment programs.  As required,  communicates with insurance companies/managed care to pre-certify patients for admission.  Arranges private payment plans with family and/or referent.  Works with team to field all inquiries, informational calls and admissions.  Completes nursing assessments, obtains orders from a provider and completes OASAS admission forms as needed.                                                           

Requirements: Must be a RN in NYS and minimum 1 year experience in Chemical Dependency, managed care or case management

 


LICENSED PRACTICAL NURSE

Location: Glenville, NY
Full time 40 hours 3p-11:30p, Full time 32 hours 7:00a-11:30p, Part time 16 hours  7a-3:30p and Per Diem all shifts (as needed)       

Collaborates with the RN in the implementation of a total health care regime including participation w/ patient and family teaching including discharge planning. Provides data to assist in the evaluation of patients’ responses to plan of care. Med pass.

Requirements: Vocational/Technical School w/ 1-3 yrs exp. Current NYS LPN license and CPR certification required. knowledge/experience.  Individual and Group counseling as well as didactic education skills required.

 


CRISIS SERVICE TECHNICIAN
Location: Glenville, NY
Full time 32 hours 7:00a-3:30p Friday, Saturday,3:00p-11:30p Sunday, Monday, and Per Diem all shifts (as needed)

Answers and screens calls to the Detox Unit, providing clerical assistance for Detox staff, physicians and managers. Assembles charts, maintains census, completes appropriate requisition for tests and replenishes supplies for Detox unit and Unit Med Rooms. Takes responsibility to accurately measure and record temperature, pulse, respirations, blood pressure, and signs of withdrawal.

Requirements: HSD/GED PLUS 1-3 yrs exp with direct patient care. Medical terminology preferred. CPR Certification

 

ADDICTION TECHNICIAN
Location: Glenville, NY
Full time, Part time and Per Diem all shifts (as needed)

Monitors, oversees, and escorts patients. Review notes and appropriate documentation on patients on a daily basis helping to ensure continuity and compliance with treatment. Provide psycho-education services. Monitors and enforces the facility policy as it relates to patient behavioral management issues.

Requirements: HSD or GED. 1-3 yrs experience in the Human Services or Health Care field, including positions requiring patient contact and general use of appropriate interpersonal skills including patient to staff boundaries and management of patient/client groups. Associates/Bachelor’s Degree may be substituted for the experience requirement. CPR is required.

 

UTILIZATION REVIEW COORDINATOR
Location: Glenville, NY                                                                                                    

To provide utilization review monitoring of the medical record and treatment planning and delivery process, comparing against managed care, ASAM, and NYS criteria. Conducts precerts, external, concurrent and retrospective reviews with insurance companies, professional review organizations, union, etc. and communicates results and changes to internal staff in a timely fashion.

Requirements: HSD/GED required. Bachelor’s degree preferred. Appropriately licensed RN, CASAC, LMHC or LMSW with a minimum 1 year of experience in Managed Care.

 

ADMISSIONS SPECIALIST
Location: Glenville, NY
Temporary (no benefits) Full time 40 hrs Monday-Friday day shift

To coordinate and perform all tasks required to prepare a patient for an admission. Greets and admits all incoming patients, family, referents and completes appropriate paperwork as needed and assigned. Meets with patient to review all data entry for accuracy and make corrections as necessary.

Requirements: 2 year/Associate’s Degree and a minimum 1 year of experience in Chemical Dependency or in an office environment.

 

PATIENT ACCOUNT SPECIALIST
Location: Glenville, NY
FT/40hrs Monday-Friday 8:00a-4:30pm

Input admissions, transfers and discharges in the computer system. Input all ancillary charges, post payments from patients and insurance companies to their respective accounts. Calculate daily census. Run daily reports.

Requirements:  HSD or GED required. 2 years related experience and/or training OR Associates in related field with 1 year experience. Experience in A/R required. Third party reimbursement and/or collections and electronic billing experience preferred.

 


SECURITY GUARD
Location: Glenville, NY
Per Diem all shifts (as needed)

Must be able to maintain and monitor security throughout facility according to procedure, exercise good judgment and discretion, and maintain a high degree of confidentiality.

Requirements: Position requires current NYS Security Guard License and valid NYS Driver’s License. HSD/GED; prior training in security, law enforcement, or related field; and 3 years pertinent experience.

 


DRIVER
Location: Glenville, NY
Full time 40 hours Monday – Friday and Per diem

Transport patients to various locations.  Be able to demonstrate ability to communicate well with patients, to write routine reports and correspondence, interpret documents, and demonstrate ability to read a map and apply basic math skills. CPR and defensive driving are required prior to hire.

Requirements: HSD or GED preferred.  MUST have a clean, valid NYS Drivers License and a minimum of 1 year experience preferred.

 


HOUSEKEEPER
Location: Glenville, NY
Full time 40 hrs 7:00a-3:30p

Performs the day to day housekeeping and laundry functions necessary to provide the goals of a clean healthy environment in accordance with the department of Health, Safety, and Departmental regulations guidelines.

Requirements
: HSD or GED preferred. A minimum of1-year experience preferred. 

 

DIETARY ASSISTANT
Location: Glenville, NY
Part time 20 hours with benefits and Per Diem all shifts (as needed)

Assist in preparation and service of all aspects of the Dietary Department. Cleans, stocks and organizes service line, coolers and storage areas. Takes inventory and assembles supplies for all unit kitchens. Follows all mandates concerning food preparation, services, storage and sanitation as set forth by the New York State Department of Health

Requirements: Less than HSD; HSD or GED preferred. Vocational/Technical in Food services a plus. No experience required, experience in food service and/or food preparation a plus. Must be able to follow Department of Health regulations and safety guidelines.

 

 

OUTPATIENT RECEPTIONIST- TROY
Full time 40 hours

Provide administrative support to the outpatient staff. Answer telephones, screens calls, and responds to requests from patients and staff. Makes and confirms appointments, schedules and coordinates special meetings.

Requirements: HSD or GED and a minimum 1 year of experience clerical experience. Must be proficient in typing and have excellent communication and computer skills required.  CPR is required within 6 months of hire.

 

 


OUTPATIENT THERAPIST or  SOCIALWORKER – TROY

Full time 40 hours

Provide comprehensive psychosocial assessment and diagnostic services to patient in an outpatient setting.  Provide individual, family or crisis counseling to a caseload of up to 35 clients. Provide comprehensive and individualized discharge planning and referral services.

 

OUTPATIENT SOCIALWORKER Requirements:  Master’s degree and LMSW or LCSW required. A minimum 1 year of experience in Case Management or Chemical Dependency Counseling. CPR certification required within 6 months of hire.          


OUTPATIENT THERAPIST Requirements:
   HSD or GED required.  4yr BA/BS in related field preferred. Master’s degree related field preferred.  A minimum 1 year of experience in Chemical Dependency Counseling. CASAC,  NBCC, CRC, LMHC or a provisional MHC. CPR certification required within 6 months of hire.

 

 

OUTPATIENT RN METHADONE COORDINATOR- LIVERPOOL
Full time 40 hours

Provide comprehensive psychosocial assessment and diagnostic services to patient in an outpatient setting. OUTPATIENT LPN- LIVERPOOL
Full time 40 hours

This is a non-traditional unique Nursing position that will prepare and dispense Methadone and Suboxone to patients accurately and timely as directed by doctor’s orders in accordance with all state and federal regulations including JCAHO. Ensures all daily dispensing records are printed and maintained.  Will after dispensing run all daily/dispensing reports, print them, have RN review and the file them so that they are available for review by state and federal agencies including JCAHO. Gathers the information needed on the Medical Screening Assessment from clients for MD/RN review to ascertain the need for a referral for a physical. 

Requirements: Vocational/Technical School. Current NYS LPN license with a minimum 1 year of experience. CPR certification required.    

 


OUTPATIENT COUNSELOR- LIVERPOOL
Full time 40 hours

The Counselor will provide comprehensive psychosocial assessment and diagnostic services to patients in an outpatient setting. Provide comprehensive case management services and coordinate services with collateral service providers. Provide individual, family or crisis counseling to a caseload of up to 35 clients.

Requirements:  HSD or GED with a CASAC-T is required.  Associates Degree in Chemical Dependency or Bachelors in related filed preferred. CPR required within 6 months of hire.

 

OUTPATIENT PEER ADVOCATE- LIVERPOOL
Part time 20 hours

Monitoring the waiting room, clinic restrooms, parking lot and grounds of the outpatient program listed above during methadone dosing hours to ensure patients adhering to program regulations. Ensuring that patients are in compliance with the Program Policy and Procedure for loitering. Supervise and manage patient flow for dosing during dosing hours and assist front office and nursing staff to ensure patients are being dosed in a timely manner. Helps to ensure participant directed care by assisting the individual to build the specific skills and relationships he or she needs in order to achieve and maintain recovery by mentoring, monitoring and motivating patients.

Requirements: High School Diploma or GED required. 2 yr/Assoc. Deg. – C.D. preferred. Secure Peer Advocate Training within 12 months. CPR certification within six months of hire.
