Conifer Park, Inc. offers a very generous paid-time-off package. Time off is pro-rated based upon date of hire. Union employees employed as of the first of the year are eligible for 9 holidays, 2 personal days, a total of 10 sick days, 7 floating holidays and 10 days of vacation. Based upon years of service, the maximum amount of vacation time off is 25 days. Part-time employees (20 hours to 31 hours), are also eligible for many benefits on a pro-rated basis.

PSYCHIATRIC NURSE PRACTITIONER
FT/40 hours

Develops and implements patient management plans, medication and therapy recommendations, records progress notes, and assists planning of continuity of care. Performs history and physical examinations on patients. Evaluates and manages detox protocols on the Detox Unit. Pre-Admission clearance for patients with medical symptoms and Pre-Admission Interviews when indicated.

Requirements: Must have current NYS License and Physician Assistant certificate from National Certification Commission /current NYS NP license. 1 – 3 years experience as a physician assistant/nurse practitioner; Experience in detoxification / addiction medicine exp a plus.

REGISTERED NURSE
Location: Glenville, NY

Performs assessments focusing on physiologic and cognitive status. Formulates a goal-directed plan of care. Evaluates effectiveness of care and patient progress toward outcomes.

Requirements: Associate Degree, current NYS RN license w/1-3 yrs exp and CPR certification required.

 

LICENSED PRACTICAL NURSE
Location: Glenville, NY
Part time and Per Diem all shifts (as needed)

Collaborates with the RN in the implementation of a total health care regime including participation w/ patient and family teaching including discharge planning. Provides data to assist in the evaluation of patients’ responses to plan of care. Med pass.

Requirements: Vocational/Technical School w/ 1-3 yrs exp. Current NYS LPN license and CPR certification required.

 

ADDICTION TECHNICIAN
Location: Glenville, NY
Full time, Part time and Per Diem all shifts (as needed)

Monitors, oversees, and escorts patients. Review notes and appropriate documentation on patients on a daily basis helping to ensure continuity and compliance with treatment. Provide psycho-education services. Monitors and enforces the facility policy as it relates to patient behavioral management issues.

Requirements: HSD or GED. 1-3 yrs experience in the Human Services or Health Care field, including positions requiring patient contact and general use of appropriate interpersonal skills including patient to staff boundaries and management of patient/client groups. Associates/Bachelor’s Degree may be substituted for the experience requirement. NOTE: some positions require CPR.

 

DIETARY ASSISTANT
FT/40hrs

Assist in preparation and service of all aspects of the Dietary Department. Cleans, stocks and organizes service line, coolers and storage areas. Takes inventory and assembles supplies for all unit kitchens. Follows all mandates concerning food preparation, services, storage and sanitation as set forth by the New York State Department of Health

Requirements: Less than HSD; HSD or GED preferred. Vocational/Technical in Food services a plus. No experience required, experience in food service and/or food preparation a plus. Must be able to follow Department of Health regulations and safety guidelines.

 

HOUSEKEEPER
FT/40hrs 12:30p-9:00p

Performs the day to day housekeeping and laundry functions necessary to provide the goals of a clean healthy environment in accordance with the department of Health, Safety, and Departmental regulations guidelines.

Requirements: HSD or GED preferred. 1 – 3 years experience preferred.

 

MAINTENANCE TECH
FT/40hrs 12:30p-9:00p

To assist in achieving safety, maintenance, improvements and operations of buildings and plant equipment by carrying out a work plan developed to provide corrective, predictive, and preventative maintenance.

Requirements: HSD or GED required. Vocational/Technical a plus. 2-3 years experience in maintenance. Ability to demonstrate appropriate skills in HVAC, plumbing, carpentry and electrical.

 

CREDENTIALING SPECIALIST
PT/20hrs

Communicates with the Directors and staff at the outpatient sites for various licenses/certifications needed to credential providers. Sends out applications to providers at Arms Acres and Conifer Park for credentialing for various payers. Reviews applications and sends them onto the various payers for individual credentialing for commercial/government payers.

Requirements: Associate’s Degree with one year of experience in billing.

 

OUTPATIENT CLINICS:

 

NURSE PRACTITIONER- GLENS FALLS
PT – 8 hours a week

Develops and implements patient management plans, medication and therapy recommendations, records progress notes, and assists planning of continuity of care. Evaluates for mental health needs.

Requirements: Must have current NYS License and Physician Assistant certificate from National Certification Commission /current NYS NP license. 1 – 3 years experience as a physician assistant/nurse practitioner; Experience in detoxification / addiction medicine exp a plus.

 

OUTPATIENT COUNSELOR – TROY
FT 32 hours

Provide comprehensive psychosocial assessment and diagnostic services to patients in an outpatient setting. Provide comprehensive case management services and coordinate services with collateral service providers. Provide individual, family or crisis counseling to a caseload of up to 35 clients.

Requirements: HSD or GED required. Must have a CASAC-T. 2yr Degree in Chemical Dependency or 4yr BA/BS in related filed preferred. 1 – 3 years experience in counseling preferred. Commitment to obtain QHP within 5 years of hire. CPR required within 6 months of hire.

 

REGISTERED NURSE-PLATTSBURGH
PT 20 hours with BENEFITS

Provide comprehensive psychosocial assessment and diagnostic services to patient in an outpatient setting. Provide comprehensive case management services and coordinate services with collateral service providers. Provide individual, family or crisis counseling to a caseload of up to 35 clients. Provide both process oriented and didactic group counseling services. Provide comprehensive and individualized discharge planning and referral services.                                                                                                      

Requirements: Current NYS RN license 1 – 3 years Case Management experience in Chemical Dependency. CPR certification required within 6 months of hire.
