Conifer Park, Inc. offers a very generous paid-time-off package. Time off is pro-rated based upon date of hire. Union employees employed as of the first of the year are eligible for 9 holidays, 2 personal days, a total of 10 sick days, 7 floating holidays and 10 days of vacation. Based upon years of service, the maximum amount of vacation time off is 25 days. Part-time employees (20 hours to 31 hours), are also eligible for many benefits on a pro-rated basis.

PHYSICIAN ASSISTANT or NURSE PRACTITIONER

Location: Glenville, NY
FT/40 hours

Develops and implements patient management plans, medication and therapy recommendations, records progress notes, and assists planning of continuity of care. Performs history and physical examinations on patients. Evaluates and manages detox protocols on the Detox Unit. Pre-Admission clearance for patients with medical symptoms and Pre-Admission Interviews when indicated.

Requirements: Must have current NYS License and Physician Assistant certificate from National Certification Commission /current NYS NP license. 1 – 3 years experience as a physician assistant/nurse practitioner; Experience in detoxification / addiction medicine exp a plus.

REGISTERED NURSE

Performs assessments focusing on physiologic and cognitive status. Formulates a goal-directed plan of care. Evaluates effectiveness of care and patient progress toward outcomes.

Requirements: Associate Degree, current NYS RN license w/1-3 yrs exp and CPR certification required.

 

ADMISSION RN
Location: Glenville, NY
FT/40  Monday-Friday  3:00p-11:30p

Assess candidates and facilitates admission into various treatment programs. As required, communicates with insurance companies/managed care to pre-certify patients for admission. Arranges private payment plans with family and/or referent. Works with team to field all inquiries, informational calls and admissions. Completes nursing assessments, obtains orders from a provider and completes OASAS admission forms as needed.

Requirements: Must be a RN in NYS and have 1-3 years experience in Chemical Dependency, managed care or case management.

 

LICENSED PRACTICAL NURSE
Location: Glenville, NY
Part time and Per Diem all shifts (as needed)

Collaborates with the RN in the implementation of a total health care regime including participation w/ patient and family teaching including discharge planning. Provides data to assist in the evaluation of patients’ responses to plan of care. Med pass.

Requirements: Vocational/Technical School w/ 1-3 yrs exp. Current NYS LPN license and CPR certification required.

 

ADDICTION SPECIALIST

Location: Glenville, NYFT/40 hours

Assumes full charge of patient case management, delivering individual and group mental health and chemical dependency services including assessment, discharge planning, managed care, and relevant communication, formulation and implementation of treatment plans, and performing specified duties.

Requirements:  Bachelor non-licensable degree and CASAC OR Master’s licensable degree w/1 yr relevant experience OR Master’s licensed degree w/QHP. 1-3 years experience in chemical dependency with mental health knowledge/experience.  Individual and Group counseling as well as didactic education skills required.

 

INTAKE REFERRAL SPECIALIST

Location: Glenville, NY
FT/40 hours Monday-Friday 8:30a-5:00p

ADDICTION TECHNICIAN

Location: Glenville, NY                                
Full time, Part time and Per Diem all shifts (as needed)

Monitors, oversees, and escorts patients. Review notes and appropriate documentation on patients on a daily basis helping to ensure continuity and compliance with treatment. Provide psycho-education services. Monitors and enforces the facility policy as it relates to patient behavioral management issues.

Requirements: HSD or GED. 1-3 yrs experience in the Human Services or Health Care field, including positions requiring patient contact and general use of appropriate interpersonal skills including patient to staff boundaries and management of patient/client groups. Associates/Bachelor’s Degree may be substituted for the experience requirement. NOTE: some positions require CPR.

 

ACTIVITIES COUNSELOR
Location: Glenville, NY                              
Full time 40hrs 8:00an-5:00pm with rotating weekends

Facilitates/supervises patients in all recreation/fitness and team programming and documents/communicates observations and progress to appropriate staff. When indicated/requested provides patients with instruction in wellness issues, relaxation techniques, healthy lifestyle and individual fitness program. Provides lecture presentations on importance of healthy leisure lifestyles in recovery. Responsible for documentation of attendance and progress in all aspects of activities programming in accordance with treatment plan recommendations.

Requirements: 4 year/ Bachelors Degree in a Certified Recreation Therapy Program, physical education or a Human Service related Degree. 1 – 3 years experience working in recreation therapy, physical education or a human service related field. Coaching experience, certification in an activities related field, and/or coursework in a sports/exercise related subject matter preferred. First Aid/CPR & Lifesaving, Life Guarding Certification required at time of hire.

 

INTAKE REFERRAL TECH

Location: Glenville, NY
FT/40 hours Tuesday – Saturday

To coordinate and perform all tasks required to prepare a patient for an admission.

Requirements: HSD/GED, Associates preferred. 1 – 3 years in Chemical Dependency or in health care office.

 

PATIENT ACCOUNT SPECIALIST
Location: Glenville, NY
FT/40hrs Monday-Friday 8:00a-4:30pm

Input admissions, transfers and discharges in the computer system. Input all ancillary charges, post payments from patients and insurance companies to their respective accounts. Calculate daily census. Run daily reports.

Requirements: HSD or GED required. 2 years related experience and/or training OR Associates in related field with 1 year experience. Experience in A/R required. Third party reimbursement and/or collections and electronic billing experience preferred.

 

RECEPTIONIST

FT/40 hrs Monday – Friday 8:00a-5:00p

Answers and screens calls. Monitors postage meter: change date daily, order meter supplies, meter outgoing mail. Sorts and distributes incoming and internal mail and packages as needed. Greets all visitors to the facility, follows confidentiality procedure and high standard of customer service.

Requirements: HSD/GED PLUS 1-3 yrs exp reception and clerical experience. Good communication skills

 

HOUSEKEEPER

Location: Glenville, NY
FT/40hrs 7:00a-3:30p – Temporary position

Performs the day to day housekeeping and laundry functions necessary to provide the goals of a clean healthy environment in accordance with the department of Health, Safety, and Departmental regulations guidelines.

Requirements: HSD or GED preferred. 1 – 3 years experience preferred.

OUTPATIENT COUNSELOR – TROY

FT 40 hours

Provide comprehensive psychosocial assessment and diagnostic services to patients in an outpatient setting. Provide comprehensive case management services and coordinate services with collateral service providers. Provide individual, family or crisis counseling to a caseload of up to 35 clients.

Requirements: HSD or GED required. Must have a CASAC-T. 2yr Degree in Chemical Dependency or 4yr BA/BS in related filed preferred. 1 – 3 years experience in counseling preferred. Commitment to obtain QHP within 5 years of hire. CPR required within 6 months of hire.

 

OUTPATIENT THERAPIST-GLENS FALLS

FT 40 hours

Provide comprehensive psychosocial assessment and diagnostic services to patient in an outpatient setting. Provide individual, family or crisis counseling to a caseload of up to 35 clients. Provide comprehensive and individualized discharge planning and referral services.

Requirements: HSD or GED required.  4yr BA/BS in related field preferred. Master’s degree related field preferred.  One to three years experience in Chemical Dependency Counseling. CASAC or LMSW or NBCC or CRC or LMHC CPR certification required within 6 months of hire.

 

NURSE PRACTITIONER- GLENS FALLS

PT – 8 hours a week

SECRETARY – GLENS FALLS
PT/20 hours with benefits

Provide administrative support to the outpatient staff. Answer telephones, screens calls, and responds to requests from patients and staff. Makes and confirms appointments, schedules and coordinates special meetings.

Requirements: HSD or GED. 1-3 years clerical experience. Must be proficient in typing and have excellent communication and computer skills required. CPR is required within 6 months of hire.

 

LICENSED PRACTICAL NURSE – PLATTSBURGH
